List View

The system starts with a list of survey titles and descriptions. Each has a link to the survey questions.

User Information

Some surveys require user identification while others are anonymous. If required users enter their data at the beginning of the survey.

Questions

Survey questions are broken by sections and are answered with check boxes, drop down menus, multiple choice selection as well as single and multi-field text input.

Results

After completing the survey the user is thanked and brought to the results page which shows total and percentage of user answers for each survey answer choice. Survey results can remain hidden.

Admin List

The administration list view, common to all GIBS applications, shows the title and status of each entry.

Admin Detail

The adminstrator creates a new survey, enters sections and writes questions. Questions can use check boxes, drop down menus, multiple choice or text entry fields. The administrator can also receive an email each time the survey is completed.
